This form is for use by Inspire Governance Services customers only.  The form is intended to capture diversity data from governors/members/trustees, in line with encouragement from the Department for Education to collect and publish Governing Board members' diversity data.  

Responses will be collated, and all reporting will be anonymised to ensure individual governors cannot be identified.  The school/academy will withhold a data set if an individual governor can be identified as a result of being the only respondent within a particular range.
